Our office hours are Monday - Friday 9:00 am - 5:00 pm.

What should I bring to my appointment?

Please bring your insurance card, referral if required by your insurer, and workers compensation authorization, if needed with you to your appointment. If you are a new patient please download the forms from this site, complete and bring with you to your appointment.

What do I do if I need to change my appointment?

Please call the office at 253-838-8377 to reschedule or cancel your appointment. We do request 24 hour notice for all cancelations or rescheduling of appointments.

What is our financial policy?

Payment is expected at the time of service unless other arrangements have been made. Payment may be made by cash, check, money order, Visa and Mastercard. We do not accept any other credit cards.

What insurance do you accept?

We accept most major insurance companies. If you would like to verify if we accept your insurance plan please call our office at 253-838-8377 prior to your appointment.
